Parents that wish to register their child(ren) for any Lourdes Athletics program must fill out the following THREE forms. Online PDF forms are no longer available.

1.  Sports Registration/Participation Form

One Registration Form needs to be filled out for EACH sport being registered. Form MUST be completely filled out.

2.  Medical Treatment Authorization Form

Only one Medical Treatment Form needs to be filled out for each child.

3.  Participation Guidelines Form

Only one Participation Guideline Form needs to be filled out for each Family.

Each sports program begins with a parent/student/coaches meeting.
